Everything you need to know about OutageReview
OutageReview is a complete investigation toolkit for root cause analysis. 5 Whys, Fishbone diagrams, timeline tracking, rigor scoring, all in a single interface. You drive the investigation; we make sure you have what you need to do it right. Unlike tools that just give you forms to fill out, OutageReview enforces investigation quality through rigor scoring, premature closure warnings, and structured workflows. Built for DevOps, IT Operations, and SRE teams that are tired of repeating the same incidents.
No. OutageReview is AI-assisted, not AI-driven. Only you can identify the true root cause of an incident. Anyone can paste logs into ChatGPT. What makes OutageReview different is that it gives you the complete toolkit to organize your thinking, apply proven methodologies, and make informed decisions.
AI helps with tedious work like parsing logs into a draft timeline. But you review it, refine it, and add context only you know. The investigation is yours. We just make it easier to do it well.
OutageReview is built for technical teams that take incident response seriously:
Quality enforcement. OutageReview is the only RCA platform that:
Most tools let you write whatever you want and close the incident. We make you do the hard work of finding actual root causes.
No. OutageReview includes guided workflows and best-practice coaching at every step. The platform teaches your team how to conduct thorough investigations, whether they're new to RCA or have decades of experience.
5-10 minutes. Create your organization, invite your team, and start logging incidents. No complex configuration required to get started.
We have three transparent pricing tiers:
All prices are per organization, not per user (up to the included seat limit).

Five proven methodologies, all in one platform:
A 0-100 score that objectively measures investigation quality based on:
Low scores trigger warnings before closing incidents, preventing shallow analysis.
Automatic alerts when you try to close an investigation that doesn't meet quality thresholds:
You can override warnings (with audit trail), but they ensure your team knows when analysis is shallow.
Deeply. We built OutageReview to adapt to your existing process - you don't restructure your workflows to fit the tool.
Custom Fields: 9 field types (text, number, date, datetime, single-select, multi-select, email, URL, phone). Set validation rules, required/optional status, and role-based visibility. Control which fields appear in search filters. Archive deprecated fields while preserving historical data.
Feature Toggles: Enable only what you use. Fishbone diagrams, 4H1W analysis, service impact tracking, change causation - each toggles independently. Disabled features are removed from the UI entirely, not just hidden behind menus.
Adaptive Rigor Scoring: Quality scoring recalculates based on enabled features. Disable Fishbone? It's not counted against you. You're measured against your defined process, not a generic template.
Process Enforcement: Configure what you track. The platform still enforces investigation rigor through premature closure warnings, phase gates, and quality thresholds - calibrated to your configuration.

PagerDuty is focused on alerting, on-call management, and incident coordination during active incidents. OutageReview is focused on post-incident analysis, conducting thorough RCAs with quality enforcement to prevent recurrence.
They're complementary tools. We plan to integrate with PagerDuty in Q1 2026 to automatically create OutageReview investigations from PagerDuty incidents.
Jira is a general-purpose project management tool that can be used for incident tracking if you build custom workflows. OutageReview is purpose-built for RCA with:
You don't have to configure anything, it works out of the box for incident management.
Incident.io and FireHydrant excel at during-incident workflows like war rooms, status pages, communication, and escalation. OutageReview focuses on after-incident workflows, with deep RCA, quality enforcement, organizational learning, and preventing recurrence.
Again, they're complementary. Use Incident.io during the fire, then move to OutageReview for thorough post-mortem analysis.
Spreadsheets and docs give you total flexibility but zero structure. With OutageReview you get:
If your RCAs are in spreadsheets, you're probably not getting the depth you need.
Absolutely. Many teams use OutageReview as their RCA enforcement layer on top of their existing incident management tools. The REST API (available on Pro and Enterprise plans) makes this integration easier with bidirectional sync capabilities.
